Information on Anal Fistulas and Fissures

Anal fistulas and fissures are a painful growth near the genitals. A fistula is an abnormal development from one cavity to another or from a hollow body cavity to the surface. The fistula can be a result of a penetrating wound like from the skin to lung, or it may be congenital like from bladder to navel or it may have formed from an ulcer or an abscess like tooth socket to sinus or appendix abscess to vagina.


When the wounds or abscesses keep getting filled by the fluids of the body cavity, it slows down and prevents healing and develops the formation of a fistula. For instance, the growth of an anal fistula can start with the inflammation of the mucus lining of the rectum. As it is constantly infected by feces, the area develops abscesses; a fistula breaks out gradually on the skin near the anus. Generally, the treatment includes an operation to drain the abscess by opening the fistula channel by operating on it so that there is no chance of recurrence.


On the other hand, anal fissures are the tears in the mucus lining of the anal canal which occur due to the passage of hard stools. Fissures become very painful with bowel movements and one may bleed with the passage of stools. The anus is the ring-like sphincter or valve at the end of the rectum and it relaxes and tightens to allow a bowel movement. The rim of this anus can have a tear, which will be called anal fissure. With every bowel movement, the anus will be stretched and fissure will be reopened causing delay in healing and manifestation of more symptoms and make the fissures chronic and a considerable misery.


Anal fissures and anal fistula are often interchanged and confused. While a fissure is a tear or crack in the anal canal, fistula develops from abscesses in the same area. Fistula is more like a tunnel between the rectum and other parts of the body, like the rectum and the vagina which can be called a recto-vaginal fistula or between the anal canal and the nearby skin.


Anal fissures mostly heal if constipation is avoided. Healing can be hastened with simple measures like stool softeners and other ointments. Steroid preparations, prescription medicines and variety of analgesics may also be used. Top medical news report that the same medication used for chest pain (angina), can be used as a treatment option for anal fissures. This medication works in such a way that the smooth muscle lining below the torn mucus layer of the anus relaxes and treating the spasm of this smooth muscle heals about 85% of the fissure. If this medication does not work, a simple outpatient surgical technique called lateral internal sphincterotomy can be used.


Anal fistula is also known as fistula-in-ano and it is an abnormal link between the interior of the anal canal or rectum and the skin surface.

A team of scientists led by Dr Lukas Van Oudenhove from the University of Leuven, in Belgium, used MRI scans to assess the emotional impact of fat when injected into the stomach.
They recruited 12 healthy-weight volunteers and scanned their brainwaves as they were shown a series of sad and neutral images as well as exposing them to sad and neutral music.

 

They then inserted either fatty acids or saline into their stomachs but didn't tell the volunteers which was which. This meant they could bypass the effects of sensory stimulation as well as any associations to childhood foods.
The scientists found the music and expressions caused people's moods to fall across the board. However, the levels of sadness among those who consumed fatty acids were around 50 per cent lower compared to those who received saline.
'Eating fat seems to make us less vulnerable to sad emotions, even if we don't know we're eating fat,' Dr Oudenhove told HealthDay.
The researchers said their findings could have important implications for people with obesity, eating disorders and depression.
However, they wrote in the Journal of Clinical Investigation, that a follow-up study would need to recruit a wider range of participants.
Dr Giovanni Cizza, of the National Institute of Diabetes and Digestive and Kidney Diseases in Bethesda, who co-authored an accompanying editorial: 'We did not know if you put fat in the stomach without pleasant stimulus, it could modulate our emotions,' he says.
'There must be a way in which the gut talks to the brain.'

The most widely prescribed antidepressants – known as SSRIs – are more likely to cause death and heart attacks, strokes, falls and seizures than the older tricyclic antidepressants (TCAs), they found.
Researchers from the universities of Nottingham and East Anglia say tricyclics may be the safest class of antidepressants for older people, partly because they are prescribed in lower doses which are just as effective.

The study, published in the British Medical Journal, analysed treatment and side effects recorded for more than 60,000 people aged 65 and over who were diagnosed with depression between 1996 and 2007.

Hundreds of young women given smear tests on the advice of GPs are having their test samples rejected by bureaucratic clinics - because they are under 25.
Patients and GPs are becoming increasingly frustrated at the zero tolerance approach by laboratories, which saw 712 women across England have tests ignored last year because of Government rules which say those under 25 do not need to be tested.
This was up from 244 rejections the previous year.

Around 3,000 women in the UK each year contract cervical cancer and it accounts for more than 900 deaths.

 

Dr Clare Gerada, of the Royal College of GPs, told BBC Newsnight: ‘I think if a GP has made a clinical decision to give a cervical smear then that smear should be processed.

The Government insist that if a woman under 25 needs a smear test they must be referred to a gynaecologist.
But one consultant told Newsnight, which compiled the data using the Freedom of Information Act, that decision was crazy.
Professor John Shepherd said: ‘I think that’s not sensible, It’s crazy. GPs are well trained doctors.
‘In most practices there will be partners who specialise in gynaecology. To send patients straight to a gynaecologist is actually a waste of man power, time and money, and not sensible.’
The age for cervical screening was raised to 25 because experts believed it caused more harm than good in younger women.
Under 25s are more likely to present falsely positive results, meaning unnecessary treatment.

NHS officials paid more than £32 each for thousands of small loaves of bread, it has emerged.
The gluten-free product, which is given out free of charge to patients with coeliac disease, costs just £2.25 in supermarkets.
Official figures show the cost to the NHS of supplying it through pharmacies has rocketed to £32.27 for a small loaf – or £2.48 a slice. The total cost of the bread was £1.25million in a year.

'I was teased for needing to borrow my wife's bras': Father describes why he had male breast reduction surgery

In 1994, on a normal day in his military job, Erik Holler discovered a lump under his left nipple.
Though thankfully not cancer, the small lump instead turned out to be the earliest stages of gynaecomastia, or male breast growth - and the beginning of a difficult condition that led to life-changing surgery.
As Holler learnt, while they're the butt of many a teasing joke, male breasts can be a debilitating condition.

Now Holler is speaking up about living with the problem and openly sharing his transformational surgery with the hope that it will help other men.
30 per cent of boys experience breast swelling during puberty and gynaecomastia in adult males is more common than many may imagine.
The most common causes of male breast growth, says ABC, are obesity, hormones and drug side-effects, but gynaecomastia differs to, say, fatty tissue gain in that the growth is the same - and as permanent - as that in women's breasts.

 
In gynaecomastia, cells grow and dirtribute themselves so that the male breast takes on the shape and size of a female's. Holler's chest began to look anything but like his own.
'The left breast actually grew more,' he told ABC's Good Morning America,  'And then the right side started growing, too. And they expanded themselves to their current condition, which, I'd probably say, I'm at least an A cup -- at least.'

But now, advances in plastic surgery are making gynaecomastia treatable. Liposuction and lazer techniques mean that men can undergo breast reduction without the heavily invasive surgery that is usually associated with the procedure.
Holler's surgery, performed by Dr. Aaron Rollins in Beverly Hills, lasted about three hours and was followed with a course of hormones to further reduce swelling.

13 per cent of all plastic surgery in the U.S. last year was on males - an increase of five per cent in just a year and a half.
And, as the internet opens the lid on gynaecomastia, more men are coming forward for breast reduction surgery than ever before. In fact, says GMA, breast reduction is one of the most common plastic surgery procedures for men.
Gynaecomastia is known to affect confidence and make an impact on body image. Holler never took his shirt off in public.
He told GMA that family taunts hurt: 'They'll say something somewhat demeaning in some ways like, 'Wow, pretty soon you're going to be borrowing your wife's bras.'
It didn't help, though, that 'man-boobs' are the subject of many a popular joke, including Seinfeld's infamous 'manzier/bro' sketch.

Dr. Julius Few, founder of the Few Institute for Aesthetic Plastic Surgery in Chicago told GMA: 'The psychosocial impact is significant.
'Often, patients are subjected to wearing multiple tee shirts or being afraid to go swimming, even in warm weather.'
All that has now changed for Holler, who recently visited the beach for the first time in years - and who now enjoys swimming with his family in the pool.
Though Holler and his family are thrilled with the results, Kathy Holler, Eric's wife, says she always found her husband sexy. His insecurity made her realise that all bodies are imperfect.
'I could see his discomfort with it,' said the mother of three. 'And it made me love him more.'
